  Hiya Druid. When you pull up your map, there should be a sidebar box that says "Add Wiki Markers." Choose your page and click "Add Marker." As far as I can tell, you can only add locations for which you have a wiki page. Hope that helps!

  Well, I'm not sure if any are simple in the way you'd like, but if you check out the "New Sheets" category at www.rpgsheets.com, there are a couple of homemade versions there. (There's also a lovely-looking fillable version of the official sheet.)
